If you are interested in collection and storage of umbilical cord blood for your child through our company, you may register:

Via phone toll-free number at 800 900 138 between the operating hours of 8am and 6pm. After this time, please leave a message on the answering machine. Messages are checked the next business day and the coordinator will call back to inform you about the following steps. This way, we ask especially mothers, whose due birth date is scheduled in the given month, in which they want to be registered, to contact us by phone - it's important to prepare everything in advance to avoid any complications in case of premature childbirth.

Through this website by completing and submitting the Registration form.

Once you register, we will send our "package for mothers” to your home address. The package contains two identical Contracts, two identical Informed Consents (of which one is for you and one you will hand over in the hospital), Questionnaires and a Receipt of Pediatric Examination (to be completed by a pediatrician during the baby’s examination at the age of six months). The instructions for parents, which are a part of this package, also contain instructions on how to proceed with these documents. Part of the package for mothers is advertising material that enables excellent bargains and discounts on products from our partners.

DOCUMENTS TO COMPLETE AND RETURN TO OUR HEADQUARTERS

The documents enclosed are: “Contract ensuring a proper umbilical cord blood collection, its further Processing and Storage”, “Informed Consent”, "Informed Consent, refferal questionnaire and a questionnaire about the umbilical cord blood sampling. We ask that you send the completed forms, one copy of the contract and the Informed Consent to our headquarters at Cord Blood Center CZ, s.r.o., Cihlářská 19, 602 00 Brno.

We are aware of the sensitive nature of the questions in the forms. However, we would like to point out that any information in the forms can be used in the future in case you or your child, upon reaching legal age, decides to donate the transplant. The questionnaire is an indispensable part of standard international procedures to donate blood, bone marrow or the umbilical cord blood.

DOCUMENTS TO BRING WITH YOU TO THE HOSPITAL

Please bring with you to the hospital one filled out and signed copy of Informed consent and hand it to the obstetrician. Upon your arrival to the hospital, please inform the hospital staff and the obstetrician about the umbilical blood sampling through our firm!

NÁVRATKA - PEDIATRICIAN´S EXAMINATION

This exam is a follow-up examination performed six months after the birth of the child. The pediatrician fills in the receipt. This document serves as a source of information that declares and monitors the health of your child in the first six months of its life. There are some illnesses and diseases that are not apparent immediately after the birth of a child but could be noticed and explicitly evident within the first six months of the child’s life. Some diseases make it impossible to further use the stem cells; therefore, it is necessary for us to have all imperative information about the child’s health. Please note that in general these diseases are rather rare in occurrence. Enclosed in this bundle is an envelope that is intended for the return of the bottom part of the examination receipt.

PROCEDURE AFTER THE CORD BLOOD SAMPLE COLLECTION

1. The personnel of the hospital informs the associate or coordinator of our company toll-free via 800 900 139 about the collection of the umbilical cord blood. In addition, it is also necessary that you use the same toll-free number to notify us about the birth and the tissue sample collection. Thus, the harvested umbilical cord blood can be immediately transferred to our tissue facility for further processing.

This toll-free line is open daily, holidays included, from 8am until 8pm. After the operating hours, you can leave a message on the answering machine. The coordinator of our company will get back to you as soon as possible and will assist you with any questions you have. The message you leave should contain: mother’s full name, the due date of the baby, the name of the hospital, who reports the collection – contact information (e.g. Nováková Andrea, 20.02.2010, hospital Třebíč, reported Novák Alexandr – phone number). The following day the coordinator of our company will get back to you and will inform you that your request is being processed.

2. The Laboratory tests usually take about 4 - 6 weeks. After completion of all necessary examinations, the Results of umbilical cord blood processing will be sent to you with all acquired sets of test results. If the test results confirm the applicability of the transplant the Certificate of quality will also be sent to you. These documents also contain all acquired test results of processed umbilical cord blood. The results and the certificate are accompanied by invoices sent by certified mail to the address stated by the mother in the contract. In case of address or name change, please inform us at once in writing or call us immediately at our toll-free number. Thank you!

The volume of the collected umbilical cord blood is a very significant variable, upon which the outcome of the transplant depends, i.e. the maximum weight of the patient determines whether or not you can use the transplant of your child (this information will be given to you after the processing of the umbilical cord blood findings, i.e. 4-6 weeks after the child birth). At the beginning none of the participating parties know how much blood will be collected from the umbilical cord. It is in the best interest of all of us that the volume of the collected sample is to the maximum amount possible. Please, get fully involved and ask your obstetricians and midwives to collect as much blood as they can; the more, the better!

3. Should you decide based on the umbilical cord blood processing results that you would like to become an owner of the transplant, you will pay for such services based on issued invoices. In order to properly process your payments, it is important that you promptly pay the due amount within the given payment schedule and that you use the correct identification codes provided to you on your invoice.

We hope that the birth of your child is without any complications. We also hope that your child will never need the transplant from the umbilical cord blood.
